Form Instructions
- Please complete the form with you and your business’ basic info, such as entity type, address, name, etc.
- Select whether you are looking for pricing for a single drone operation (“Individual Pricing”) or coverage for multiple drones (“Group/Blanket Pricing”).
- Answer any Yes/No questions by selecting the applicable checkbox.
- Please select any operations that apply to your business… events, agriculture, law enforcement, etc.
- Please list the info for each drone you operate, including Make, Model, Serial #, Value, Flight Hours, and select whether you want physical damage coverage for the drone. Please note: If you need additional drones insured than there is space on the form, please email that info to us at info@whiteoakis.com
- Please list all operators and related certification information.
- Please list your desired liability coverage limits. Standard limits are $1,000,000 per occurrence/$2,000,000 aggregate limit; however, you can list higher or lower limits.
- When signing the form, please remove “Form Participant” and type your name. See below.
